An umbrella review of prediction tools for medication-related harm found no systems ready for routine clinical use. Researchers analyzed multiple studies to evaluate how these tools identify high-risk adult patients. While various models exist, inconsistent performance and development gaps persist. Practitioners cannot yet rely on these tools for daily hospital triage.
